4. Extract from the State Register of Legal Entities;
6. Copy of identity card;
8. Agreement to the processing of personal data, where the tender is submitted by a natural person, model;
10. Power of attorney, in the case of a bid submitted through a representative;
12. Date and signature of the assignee or representative as applicable.
